Deadline for application submission is on or before 3 p.m. Friday, January 23, 2026.
Instructions for completing the production form: Production is calculated by totaling the sales price(s) not compensation.
MDC applications submitted after the 24th time/date deadline and on or before the extended time/date deadline of the 31st, will incur a $50 late fee.
